  • From Palm Beaches or Treasure Coast Florida traveling to specific providers in Florida, Illinois, Maryland, New Jersey, New York, Pennsylvania or Washington, D.C.. (see below)

Please make an appointment at one of the partner clinics listed on EMA's website. After your appointment is made please complete their intake form for funding assistance. Please allow up to 48 hours for EMA to get back to you.
